Our therapists treat adults who need help with many types of conditions. To refer a patient, call
1-800-365-3548. We are here to help any time of the day or night, seven days a week, 24 hours a day. Our physician-led team of clinicians will evaluate each patient and develop a personalized treatment plan.
Scheduling Patient Appointments
To schedule an appointment, we need the following information about your patient:
- Name
- Phone number
- Date of birth
- Insurance information
- Referral reason
