Atrium Health Floyd-Polk Foundation
Atrium Health Floyd-Polk Foundation was founded in 2021 as part of the strategic combination between Floyd Medical Center and Atrium Health. As a 501(c)(3), not-for-profit corporation, the Foundation manages an endowment of more than $160 million. Income from the endowment may be used to make long-term investments in the communities served by Atrium Health Floyd to address social determinants of health, such as food, housing and transportation, and to alleviate disparities in health care and health access.
This new foundation is not affiliated with Floyd Healthcare Foundation, which will continue in its role to raise friends and funds to support Atrium Health Floyd Medical Center.

The mission of the Atrium Health Floyd-Polk Foundation is to serve as a trusted regional resource partner by investing in community organizations to drive transformational change in health and well-being for all.
Atrium Health Floyd-Polk Foundation supports creative approaches to address long-standing challenges, focusing on resolving the root causes of issues rather than immediate needs.
Grants
Foundation grants will be made to a wide range of programs that strive to enhance the health of the community. Applications for the current grant cycle will be accepted from January 25 - February 23.
Grant decisions will be announced on May 31.
Read all the details below before submitting your grant application.
Consideration will be given to programs that specifically address one of these areas:
- Mental health, behavioral health or substance abuse, such as teen suicide and parents experiencing mental health concerns
- Access to care and mobility
- Educational opportunities
- Food access and security
- Housing
Preference will be given to projects and programs that:
- Support the scaling of a proven program
- Show collaboration
- Show the ability to mobilize diversified financial support and in-kind support and volunteers to sustain the program
- Are based on proven evidence-based research methods for the project and the interventions
- Offer innovative strategies and creative solutions without duplicating services
- Demonstrate past performance in delivering quality service to the residents of more than one county in the service area
Funding will not be considered for the following activities:
- Retroactive funding (project dates must occur after May 2024)
- Annual fundraising campaigns
- Advocacy or lobbying efforts
- Grants for religious activities
- Endowments
- Grants to individuals
- Debt or deficit reduction
- International NGOs
- Academic research
- Grants for re-granting
2024 Grant Cycle Timetable
- January 25: Grant cycle opens
- February 23: Grant proposal deadline
- April: Site visits scheduled
- May 31: Grant decisions announced
Eligibility
Non-profit entities, public charities and educational institutions serving the citizens of Floyd, Polk and Chattooga counties in Georgia and Cherokee County in Alabama are eligible to apply for grant funds. Governmental agencies, and religious and civic organizations will be considered if the grant is to be used strictly for charitable purposes as defined by the Internal Revenue Code, Section 170.2. An organization may submit only one proposal per grant cycle.
Requirements
- Current IRS determination letter
- Registered as a nonprofit with the Georgia or Alabama Secretary of State
- Service to citizens of Floyd, Polk and Chattooga Counties in Georgia and/or Cherokee County in Alabama
- Board of directors with representation from the community/communities served
- Annual plan and budget
- Audited financial statements for the last fiscal year if annual budget is greater than $500,000, or a review by an independent, certified public accountant if annual budget is less
- Documentation of some matching support
Reporting
Grants are awarded for a 12-month period, June through May. All successful grant recipients are required to provide the Foundation with a final report no later than 30 days after the 12-month grant period. Report guidelines and a copy of the report form will be furnished at the time the grant is made.
